Trump announces $90 one-time payment for 20M Medicare seniors

US President Donald Trump has announced a one-time payment of $90 for more than 20 million Americans enrolled in Medicare. The payment is meant to help seniors cover part of their monthly health insurance premiums.
Trump announced the plan in a Truth Social post on Friday night, October 2. The White House said the money will come from the Medicare Improvement Fund, a fund Congress set up with about $2 billion.
According to the Centers for Medicare and Medicaid Services, about 20.8 million beneficiaries are eligible. Most will receive the money by direct deposit around October 8. Those without direct deposit details on file will get a paper cheque later in October.
Not everyone in Medicare will get the payment. The White House said people in Medicare Advantage plans, those whose premiums are paid through Medicaid, and higher-income earners who pay an income-related monthly adjustment are not eligible.
The standard Medicare Part B premium is $202.90 a month for 2026. That means $90 covers less than half of one month’s premium.
The payment is a one-time rebate, not a permanent premium cut. It comes one month before the November 3 midterm elections, in which control of Congress is at stake.
