Taylor Swift gave her fans exactly what they predicted, premiering the music video for her new single “Patient Zero” at the 2026 MTV Video Music Awards on Sunday. Reddit fan theories had correctly called the surprise debut days before the show.

Swift premiered the self-directed video after accepting the inaugural Artist Director Honors award at the Peacock Theater in Los Angeles, where she was introduced by Dakota Johnson. The cinematic video stars Johnson and Colin Farrell alongside Swift, with a cameo from Cara Delevingne, and was shot by Oscar-winning cinematographer Emmanuel Lubezki.

In the video, Swift appears as the ghost of Farrell’s former lover, silently warning Johnson’s character as she falls into the same toxic relationship. The two women twin in matching outfits as the story reveals that Farrell’s character has been secretly poisoning them both. Swift told fans she wanted to blend gothic ghost stories, true crime and psychological thrillers.

“Patient Zero” is one of four new tracks on the deluxe edition of her 12th studio album, “The Life of a Showgirl: The Encore,” alongside “Cleveland!”, “Pink Clouding” and “Babylon.” Swift had teased the video with a 13-second clip on Instagram on September 24.
