Explained: Trump’s $90 Medicare checks — who gets them and when

President Donald Trump has announced a one-time $90 payment for more than 20 million Americans on Medicare. Here is what is known about who gets the money, and when.
Who gets it: About 20.8 million beneficiaries enrolled in Original Medicare Part B, according to the Centers for Medicare and Medicaid Services. The payment is automatic — no application is needed.
Who does not: People in Medicare Advantage plans, those whose premiums are paid through Medicaid, and higher-income earners who pay an income-related monthly adjustment amount (IRMAA) are excluded.
When: Most eligible beneficiaries will receive the money by direct deposit around October 8. Those without direct deposit details on file will get a paper cheque later in October. Payment status can be checked with the Social Security Administration from October 15.
Where the money comes from: The Medicare Improvement Fund, which Congress funded with about $2 billion to improve the fee-for-service programme. The standard Part B premium is $202.90 a month for 2026, so $90 covers less than half of one month.
The payment is a one-time rebate, not a permanent premium cut. Seniors should beware of scams: no one should call asking for bank details to “release” the money.
